Cancer Navigator

By becoming a Pathway Navigator, you will be part of a community of similar roles across Greater Manchester. Working closely with the relevant Clinical Nurse Specialist (CNS), you will have a key role in ensuring that patients have a cancer diagnosis as early as possible, supporting those patients in the early stages of the pathway by being their first point of contact up until diagnosis. - ordinating the appointments and information required by clinicians in the pathway to deliver the faster diagnosis standard.

The post holder will support the implementation of personalised Care for people affected by working with the clinical nurse specialists when implementing personalised Care Interventions including:

- assisting with and/or delivering holistic needs assessments and producing associated care and support plans
- Personalised Stratified Follow-up, e., risk-stratified remote monitoring and surveillance
- Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist need
