We are currently seeking a highly motivated and organised Community Outreach Worker to join our friendly and supportive Community Outreach Team.

**About the Role**:
As a Community Outreach Worker, you will play a vital role in supporting the care management process. You will work with adults with a variety of needs, promoting independence, health, and wellbeing. Your work will help individuals improve their quality of life and build meaningful connections within their communities.

**Key responsibilities include**:
Supporting individuals to access services and community resources.

Providing advice, information, and signposting to appropriate agencies.

Facilitating involvement in local and wider community activities.

Contributing to care planning and review processes.

**We are looking for someone with**:
Excellent interpersonal and organisational skills.

Experience and knowledge of the care planning and review process.

A sound understanding of Care Management, Community Care principles, and relevant legislation.

A relevant qualification such as NVQ Level 3 in Care / Promoting Independence, or equivalent.
