We have Healthcare Support Worker vacancies within the Adult Community Services Department to provide support to the Community Staff Nurses with the provision of nursing care to allocated patients within their homes. Homerton University Hospital is a fully integrated service with Adult Community Nursing. The aim of our organisation is to provide a first class healthcare service across the in-patient and community services. You will be primarily based at one of our community service health centres within City & Hackney and will be expected to travel to community care sites as appropriate. The post holder will be responsible for providing delegated basic nursing care duties to allocated patients in their own homes and/or a variety of settings. The post holder will work within a team for a defined number of patients, and will work under the supervision of the Deputy District Nursing Team Leader, and lead by the Team Leader. You will work closely with the Community Staff Nurses in their team and the aligned GP’s with the team. They report their finding to the trained staff for advice and support the patients in communicating their needs to the team. **Main duties include**: - The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings. - Implementing and monitoring their care needs under the supervision of community registered nurses. - They will be responsible for delivering a high standard of holistic nursing care whilst promoting and maintaining health education and independence. - The post holder will take responsibility for their own actions when delivering delegated clinical care; maintaining competency and skill in line with role requirement. - Undertakes delegated clinical / technical tasks following competency assessment (i.e. catheter care, pressure ulcer prevention, wound care, sample collection). - To undertake delegated clinical observations including vital signs, blood sugar, skin integrity assessment. - To identify changes in patients’ condition and, within, their capabilities, recognize where significant changes in the care plan are required and report back to senior nurses in a prompt and timely manner. - To instigate appropriate emergency community procedure when faced with a deteriorating patient. - To provide high quality care in patient’s homes or clinic settings in line with current evidence-based practice and Trust Policy. - Undertake personal care duties in the home setting e.g.: maintaining patient’s hygiene, toileting **Please note that applicants must have the right to work in the UK, without the need for sponsorship** Homerton Healthcare NHS Foundation Trust is a high performing and flourishing organisation serving the population of Hackney, East London, The City and beyond.
