We have an opportunity for someone to become a Team Leader with us within our supported living services in the Five Towns area of Wakefield

**We will provide you with**
- **£13.73 per hour (£26,773.50 per year) **on a 37.5 hour per week contract

As a Team Leader, you will lead a team of support workers to deliver a bespoke and person-centred support plan for individuals with a learning disability, that may display behaviours that challenge and complex needs.

You will play a lead role in ensuring a therapeutic environment is provided and maintained that promotes choice, wellbeing and the promotion of an independent lifestyle for the People We Support. You will also play a key role in developing the potential of our support workers and helping reach their career goals.

At Millennium, we support vulnerable adults that may have a learning disability, autism, other complex needs, and may also display behaviours of distress. Starting as one service in Featherstone in 1998, we are now a growing and forward-thinking organisation that strives to make a positive mark on the Health and Social Care sector. We operate services across the Yorkshire and Warwickshire area that specialise in supporting adults with learning disabilities, Autism and complex trauma on their journeys through life and in the community. From transitioning from hospital to a safe new home, to fostering friendships and building skills, we offer a person-centred support, tailored to support individuals no matter their circumstances.

We work in line with the **Transforming Care Agenda** and will support you to provide **Positive Behaviour Support**. This means the People We Support are provided with bespoke and person-centred support that will provide them with the foundations to gain further independence

**What the job involves, day to day**
- Promoting a safe and inclusive environment for both the People We Support and the People We Employ
- Creating and managing rotas to ensure safe staffing levels in the service
- Supporting the ongoing development of support plans, risk assessments and behavioural plans
- Supporting the HR and recruitment process to ensure the needs of the People We Support are met
- Supporting the service to maintain regulatory requirements, ensuring policies and procedures are followed
- Supporting the service manager to undertake internal audits and compliance reports

**What the job involves day to day**
- Having a clear presence in the service and promoting a safe and open environment for both the People we Support and People We Employ
- Creating and managing rotas to ensure safe staffing levels are present in the service
- Supporting the ongoing development of service plans including support plans, risk assessments and behavioural plans
- Developing and maintaining positive partnerships with the People We Support and their own network of support
- Supporting the HR and Recruitment process to ensure the needs of the People We Support are met
- Communicating and regular reporting to the service manager
- Ensuring the environment of the People We Support is inclusive, safe and person centred
- Supporting the service to maintain regulatory requirements and policies and procedures are being followed
- Under the direction of the service manager, undertake internal audits and compliance reports
