Support Worker

**Overall Responsibility**:
To effectively deliver a supported living package based upon structured key work sessions to vulnerable young people aged 16 - 18.
Promote the development of the life skills required by young people in order to successfully support their transition to independent living.
**Key Areas of Responsibility**:
Management and ongoing assessment of risk and need of a varied caseload of young people living semi-independently in the community.
Work with the Senior Support Worker, Home Manager and young people to develop a structured programme of key work sessions based upon the young person’s individual needs and focused on developing their independent living and transition into adulthood. All key working sessions to be consistent with the Individual Placement Agreement.
Prepare individual key working reports regarding each young person’s progress, these reports must be completed to a high standard for submission to the placing authority and other relevant partner agencies.
Monitor the young person’s ability to engage with everyday living and develop skills where necessary. Escalate any concerns to the relevant agencies/professionals.
Promote safe, consistent and appropriate boundaries between staff and young people in
conjunction with the ‘Young Person’s Agreement’.
Encourage the young person to actively engage in the local community, building positive
relationships and maintaining social networks.
Work in partnership with external agencies and support the young people to attend meetings with other professionals as and when required.
Maintain confidentiality for young people in line with GDPR Legislation and report any concerns regarding young person’s welfare to the Home Manager and/or the Safeguarding Lead.
Attend mandatory staff meetings and training sessions as and when required. Be responsible for your own continued professional development.
You will be expected to take part in regular structured supervision sessions.
The post holder must be flexible and willing to provide a service outside of normal office hours.
You will be expected to work some evening and weekend shifts as required, plus a minimum of one sleep in per week.
To undertake any other relevant and appropriate duties as directed by the company.

**Job Types**: Full-time, Part-time, Fixed term contract, Temporary contract
Contract length: 6 months
Part-time hours: 40 - 60 per week
**Salary**: £9.50-£12.50 per hour
