This is an exciting opportunity to join the Cancer Services team as a Diagnostic tracker. The post-holder will support the booking and escalation processes for suspected cancer patient pathways, acting as the contact point between the Cancer Services Department and the diagnostic services of Pathology and Endoscopy

This is a rewarding opportunity to join the Cancer Services team. The Cancer Diagnostic Tracker is responsible for collecting and recording data on the Cancer Information System relating to diagnostic tests and results. You will need to follow escalation procedures within the Pathology and Endoscopy departments

1. To ensure that the Trust has robust information to monitor diagnostic waiting times relating to patients on a cancer pathway by: 1. Tracking the progress of patients by identifying key points in the patient journey from Trust systems.
2. Obtain key information from other Trust staff including secretaries, MDT Coordinators, Booking Clerks, etc.
3. Obtaining key items of data from other systems and entering this key information into the Cancer Information System
4. Creating management summaries for high-risk patients approaching agreed thresholds of waiting time
5. Liaising with service managers and MDT Coordinators in order to prevent breaches of agreed waiting time
6. Highlighting high risk patients to the Cancer Services and Diagnostic Services Management Teams
7. Assisting with the production and maintenance of Patient Tracking Lists (PTLs) relating to diagnostics

1. To collate the information required for national datasets such as CWT, COSD, SACT, etc.: 1. Ensuring all data items required are captured in the trusts Cancer Information System
2. Ensuring data is collected in a timely fashion and is available for upload nationally within national deadlines
3. Summarise patient pathways for review and analysis
4. Highlight potential blockages in the system
1. To assist in the provision of both routine and ad-hoc information to the Trust Board, Directorate Managers, Clinicians, and Commissioners: 1. Supplying information for inclusion in key reports
2. Providing ad-hoc information requests pertaining to diagnostics
3. Assisting in the development of information reports from the Trusts Cancer Information System
4. Audit Cancer Diagnostic Pathways as and when requested to understand peaks and troughs in waiting times and areas that require additional focus.

**1. To ensure that data is robust and fit for purpose by**: 1. Ensuring all data is validated prior to its inclusion in any reports entry onto the database
2. Liaising with relevant data quality leads in order to highlight any problem areas
3. Providing both routine and ad-hoc validation checks on data held on trust systems
1. To ensure all procedures are fully documented and that all work undertaken complies with both Data Protection and Caldicott recommendations
2. To support timely booking of endoscopy investigations as and when required
