**Reconnect Assertive**
**Outreach Worker** to work across the Surrey area
**Salary**: £26,000 - £27,000 Per Annum
**Hours**: 35 hours

**Location** - Guildford with travel across some of the Surrey area
The aim of the Reconnect service is to provide care after custody of people leaving prison. Working in Partnership with Surrey and Boarders Partnership Foundation Trust and Barnardo’s, providing liaison and diversion services to the Surrey area.
Surrey will be utilising its Liaison and Diversion Service (L&D) contract to deliver Reconnect Services. With the L&D Service, people of all ages passing through the criminal justice system are assessed and those with mental health, learning disability, substance misuse and other vulnerabilities are identified as soon as possible in the justice pathway. Identified individuals are provided with and supported to access appropriate services including, not limited to, mental and physical healthcare, social care, substance misuse treatment and safeguarding.
You will work with various colleague and the probation service and act as an advocate and care navigators to ensure the clients are transitioned to the community effectively from prison. Supporting them to find employment, undertaking training, access support services and cultivate positive social networks. Helping to engage with key local services to improve their health and social care outcomes.
**Our ideal applicant will have/be;**
- Previous community experience of drug, alcohol, mental health support work
- A working knowledge of supporting clients to access services such as: hostels, housing, benefits, primary care, mental health, physical health and treatment services
- Ability to work with complex clients in the community
- Ability to travel to and from a number of different locations on a daily basis
- An understanding of Safeguarding policy and practise
- Extensive knowledge of and understanding of mental health issues
- Good communication skills both written and verbal
- Proficient in Microsoft Word and Excel
