Our Urgent Care Hub is a collaboration of our CRHTT and our Haven team working together. These services are integral to our Urgent Care Pathway and ensure people presenting to acute mental health services receive a compassionate, person
- centred assessment and timely care plan whether this occurs in The Haven, CRHTT or in the person’s home
I am looking to recruit skilled and highly motivated practitioners to join us and develop excellent working relationships with the emergency services who refer people to us.
You will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing either/both The Haven and CRHTT services with a particular focus on:
- Utilising your excellent clinical skills and judgement to assess
- Demonstrate an understanding of clinical interventions relevant to care group to include education, care plans and discharge planning
- Work compassionately and creatively to identify therapeutic and evidence-based outcomes, care plans for people who are experiencing a crisis
- Contribute to developing bespoke crisis plans and work closely with our inpatient and CMHT.
- Work closely with carers, families and partner agencies to facilitate and promote safety and well-being of service users and their families/carers.
Covid-19 full vaccination may be a condition of employment for this role, unless an exemption applies. The successful applicant may have contact with vulnerable service users and we will therefore seek proof of being fully vaccinated during the recruitment process.
We provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire.

This is an exciting and dynamic role that offers the opportunity for development and implementation of high-quality services. Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.
As our Coastal Urgent Care Pathway moves towards CORE 24 standards, the post holder would need to work flexibly to meet the needs of the service which could include working unsociable hours within a 24/7 service. This role supports the work of and across Urgent Care Teams (CRHTT, The Haven, Mental Health Liaison team and the Liaison Suite at Worthing General Hospital) and as such the post holder may be expected to work across these teams as required.
Working unsocial hours does bring a salary increase, in line with Agenda for Change guidelines.
