1. To provide an effective, customer friendly and efficient service to clients presenting with a history of homelessness and rough sleeping, developing individual support plans and working with other agencies to address multiple and complex needs.
2. To advise clients on a full range of options to resolve their housing problem including intervention if threatened with homelessness, registering for social housing, private sector access schemes, relevant referrals for supported accommodation and advice on possession proceedings and mortgage repossession and welfare benefits where appropriate.
3. To liaise with social and private landlords in order to agree tenancy support plans in order to enable vulnerable persons, including rough sleepers and those with a history of failed tenancies, to access and sustain suitable accommodation.
4. To promote a holistic approach to housing options and homeless advice in order to provide tenancy-related support and promote a partnership approach to resolving an applicant’s housing needs and support needs.

You will have a critical role in providing specialist tenancy related support to vulnerable people who are at risk of becoming homeless. You will work with service users with complex mental health needs to live independently and remain in their own home.
You will act as the main point of contact for tenants and landlords in order to answer any queries and troubleshoot any issues throughout the life of tenancies and also act as the conduit between MPFT Mental Health services and the local authorities.
You will assist in the delivery of a high standard of support to service users that have been identified as vulnerable and at risk of losing their home.
You will be responsible for working in partnership with agencies external to the Trust supporting service users to maintain their independence within their preferred place of residency in the independent and non-statutory sector.

**Things that you will have**:
