Carers Support West Sussex provides services for and with unpaid carers who look after a relative or friend who could not manage without their support. Our teams provide tailored information, advice and support to help improve carers’ resilience and ensure they are supported to maintain their caring roles.
Using experience of working with adults and families, a working knowledge and / or lived experience of social care and health, the post holder will use excellent communication and interpersonal skills to offer carers one-to-one support either face to face, via the telephone or using virtual video mediums such as Zoom or MS Teams.
This role is funded by UK Government Household Support Fund and is a fixed term contract to end July 2026. You will be predominantly working from home and will be readily able to travel across the West Sussex, predominantly in Worthing, Adur and Arun for team meetings and carer events.
**Job Role**:
- Work as part of the community team which offers support for and with carers across the County of West Sussex, covering 3 Districts.
- Provide carers who have identified as struggling financially, with tailored, personalised information, guidance, emotional and practical support on a wide range of issues based on the principles of trust, respect and dignity.
- Use the Carers StarTM as a framework to support carers identified as struggling financially, to create a personal plan to identify and achieve positive change. Use active listening skills to empower carers to communicate issues important to them and set their own priorities.
- Research and explore options to empower carers to make informed decisions and find solutions which work for them to achieve their goals.
- As appropriate and where there is an unmet need, support carers via a Preventative Assessment to access health and wellbeing grants and equipment.
- Demonstrate the impact the service has made through Carer Star reviews, surveys, case studies and collating general feedback.
- Liaise and work with colleagues from across the organisation in response to specific carers’ needs ensuring carers have a positive journey with us, minimising duplication and making timely and appropriate referrals to other internal services.
- Accurately record all interventions on the Client Record Management System to enable timely and informative reports on the services provided, ensure decisions are evidenced and all actions recorded.
- Work as part of the community team to provide support as needed to ensure high quality services are delivered to and with carers.
- Work collaboratively with Health, Social Care and Voluntary Sector partners to refer and signpost carers for targeted and specialist support
- Promote the principles of Think Carer across all networks.
- Work within Carer Support West Sussex’s Policies and Procedures, being mindful of confidentiality, data protection, equality and diversity in all areas of work. Follow processes and procedures to ensure that adults and children are safeguarded and the health and safety for staff, volunteers and carers is considered.
