Humber Teaching NHS Foundation Trust’s Award winning YOURhealth service are looking for an enthusiastic, dedicated individual who is passionate about helping / supporting people to become healthier and change their lifestyle within the East Riding of Yorkshire as a Social Prescriber. The post advertised will be to working with our existing team of dedicated social prescribers in several locations inthe East Riding, Bridlington, Goole, Withernsea.
Community Link workers give one-to-one and group support, help people to overcome barriers to improve their lifestyle and provide on-going encouragement and motivation.
You will also be expected to promote and market healthy lifestyle issues within the local communities to encourage, change within areas of deprivation and isolation.
Excellent interpersonal communication skills are an essential requirement of Community Link Workers they will be working with individuals (and occasionally groups of people) from a variety of backgrounds and have a sound understanding of behaviour change techniques and asset based community development.
The post holder must be flexible to work daytime and evenings, including weekends. You must also be required to commute across the geographical boundaries of East Yorkshire.
