Hospital Discharge Support Worker

Libertas Reablement Service are looking for new employees who are highly motivated and with a background in care for people to work with our new and exciting Home from Hospital team, covering Lincolnshire. This will involve with supporting discharge out in the community and visiting the person in their home to ensure they have everything they need to be comfortable, offering practical and social support, personal care and signposting to other services where appropriate.
**Job Title**: Hospital Discharge Support Worker
**Responsibilities**:
- Provide a versatile and responsive service to optimize reablement opportunities in the community.
- Form part of an Integrated Care Pathway for patients, preventing hospital admission and supporting discharge.
- Work with patients in their homes, providing necessary support.
- Identify additional support needed for patients being discharged from the hospital.
- Keep accurate and confidential records of client information and needs.
- Liaise with patient carers/family members as necessary.
**Requirements**:
- Driver with access to own vehicle.
- Experience of working in care is essential.
- Flexible working patterns, including weekends.
- Full-time position with full-time day and evening shifts.
**Salary and Benefits**:
- £12.50 per hour plus paid travel time.

- Mileage paid at 30p per mile.
- MOT paid.
- Uniform and PPE provided.

- Contracted hours offered after probation.
