Community Support Worker

Working to support adults and their families who have complex social needs and are more vulnerable as a result of disability, exclusion, or diminished capacity, you'll maximise their potential to live independently.
You will assess social care needs using a person centred and strengths-based approach and contribute to the development of care and support plans. The role will require you to manage a caseload, including assessment of risk through direct work, within aquality assurance process. A focus of the role will be to ensure vulnerable people are safeguarded in line with ECC's statutory obligations.
**The Team**
You will be working closely with the social workers within the team, and the wider MDT within Brockfield House.
The role will involve working from Brockfield House, working with the patients, both male and female across the seven wards. Assisting patients with debts, benefits, and any other practical issues they may have to ensure that they are equipped for dischargeand integration back into the community.
Your role will also be working closely with family/carers. Also in helping to facilitate visits within the unit.
You will also work closely with the Specialist Forensic Community Team while facilitating discharge into the community.

**About You**
You'll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis. You will have the drive and passion to want to make a difference, and a willingness to learn. Ideally, you will have some experienceof activity-based work with adults and some knowledge of legislative and policy guidance applicable within social care settings. Knowledge of secure service settings and mental health services is desirable.
This role involves travelling around Essex. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.
As part of employment with Essex County Council (ECC) should your role require a DBS check you will be required to register with the DBS update service.
