** The Role**
As a **Care Coordinator**, you will play a key role in ensuring the smooth and efficient delivery of care services. This role is ideal for someone with care experience who is highly organised and enjoys problem-solving while building strong relationships with clients and care teams.
** Key Responsibilities**
- Oversee and coordinate live-in care packages, ensuring high standards of person-centred care.
- Plan and manage caregiver rotas, including annual leave and schedule adjustments, to ensure continuous and reliable care delivery.
- Lead, support and mentor a team of caregivers, promoting ongoing professional development and wellbeing.
- Act as a primary point of contact for clients, families and care professionals, maintaining positive and supportive communication.
- Match caregivers and clients appropriately to ensure personal compatibility and the right level of skills and experience.
- Conduct assessments and reviews to ensure care plans are being followed and outcomes are being achieved.
- Maintain accurate and up-to-date client and caregiver records within our internal systems.
- Support the recruitment process by interviewing caregivers as needed.
- Ensure full compliance with regulatory standards and company policies.
- Confident using IT systems, including Excel and our internal scheduling platform (Nourish).
What’s on offer**
- £28,650 per annum
. 8.30am to 5pm Monday to Friday / On call once in every 4 weeks
- 40 hour contract
