We are looking to recruit a highly skilled Enhanced Clinical Practitioner to deliver care to the residents of Dorset. This is an opportunity to be involved in a truly integrated way of working to deliver high quality proactive health care.

You will be joining a multi-disciplinary service to provide rapid response and admission avoidance to patients, working with the team in service delivery, triaging and prioritising visits to achieve overall objectives. You will also deliver some proactive preventative interventions.

You will be based at St Leonards Integrated Urgent Care Hub. You will join an excellent team of Practitioners. The Hub offers a truly integrated service with Social Services supported by Health and Social care co-ordinator's, rehabilitation services, Community Nursing teams and Older People's Mental Health services.

You will provide clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and decision-making skills. Your role will be community based.

The Service must be covered 8.00 to 20.00 with a weekend rota as the networks develop.

To provide high-quality person-centred care and physical assessment which always considers people’s safety, privacy and dignity using enhanced clinical knowledge that is evidence based.

To undertake independent assessment and management of service users to incorporate pharmacological considerations, where appropriate making complex clinical decisions regarding service user management and clinical outcomes.

To provide enhanced clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and enhanced decision-making skills.

To manage change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research, and education. To promote and demonstrate best practice by integrating evidence into practice.
