We have an exciting opportunity within our Urgent Response and Hospital and Care At Home Team

We are looking for two full time Nursing Advanced Clinical Practitioners

The Urgent Response Team (URT) is a multidisciplinary team of advanced clinical practitioners, nurses, therapists, social workers who deliver urgent response services to Salford residents.

We accept referrals from GPs, NWAS, and multiple other health care professionals. The fundamentals of the service are to enable patients to remain in the own homes and prevents unnecessary ED attendances or non-elective admissions by providing a community emergency service for up to 72 hours for adults in their own home.

The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Virtual Wards pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of Salford and deliver an NHS national priority.

The team provide a 7 day response to referrals during the hours of 08:00-22:00

**As an Advanced Clinical Practitioner you will**:

- Undertake a significant clinical caseload to an advanced professional standard using specialist knowledge from theoretical and practical experience
- Develop an extended scope of practice beyond their primary profession
- Triage referrals mostly independently or with senior ACP or medical colleagues and make onward referrals
- Support redesign of the flow of patient referrals from Primary care to secondary care
- Be responsible for the day to day medical management of patients on the Urgent response caseload.
