Caradon Integrated Community Mental Health team is a progressive and recovery focused team with a strong emphasis on facilitating high quality, effective and evidence based interventions with people who experience severe and enduring mental illness. We are a team of highly motivated and supportive professionals who work to ensure we empower individuals to independently manage their own mental wellbeing.

We are looking for a Senior Mental Health Practitioner who is a confident and motivated with excellent communication skills, a professional background in mental health nursing, occupational therapy or social work; you will demonstrate clinical practice that is consistent with the vision and values of our Trust.

As an Integrated Community Mental Health Team member you will have both a positive outlook and sense of humour to manage the challenges of working in a busy locality that extends to seaside towns and rural locations. You will have knowledge of general mental health problems, and the impact this may have on individuals and their treatment. You will have knowledge of the CPA process and relevant mental health legislation. You will demonstrate confidence in the delivery of assessment and treatment interventions/therapy; you will be enthusiastic, passionate and driven to work within a team focused on recovery.

**Main duties of the job**:
To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
