Following required assessment start planning patients out of hospital journey on admission understanding the patients individual needs, their normal level of ability including their social background to establish robust plans of care. This will involve working as part of the multidisciplinary team (MDT) within frailty, urgent care and acute medicine. Acting as an effective role model in discharge supporting all members of the MDT to progress the patients journey. Complete initial conversations to identify patients needing support at home and support ward staff to complete Early Discharge notifications Identify Complex patients and handover to clinical team members Identify Medically Optimised Patients Allocate Pathways on Cerner + Optica Update ADDs Initiate Needs Based Assessments + ensure completed 48 hours prior to ADD Attend daily board rounds Document confirmation of Packages of care on Cerner + Optica Ensure appropriate referrals sent 48 hours prior to ADD Attend + Contribute to weekly MDT meetings You will need to have a good understanding and overview of the work of the hospital frailty, dementia and discharge teams to fully support them and to be able to work autonomously in their absence, using initiative to prioritise and manage the workload in an informed and effective manner.
