Dorset HealthCare is seeking to welcome a new community consultant geriatrician, or a doctor with a specialist interest and extensive experience in caring for people with increasing frailty, to our team. This is a part time post of 7.5 sessions per week with Dorset Healthcare, with further sessions likely to be available with Dorset County Hospital. The West Dorset Locality (Dorchester, Weymouth and Bridport) is a proactive integrated ‘forward thinking’ team, who work together to achieve true integration, always having the patient at its centre. The team provide assessments and treatments for inpatients on our wards. They support our community teams by undertaking domiciliary assessments and participating in a virtual ward management of local patients, which is integral to the local service that supports frail and elderly patients to remain in their own home. And you’ll be working towards our vision to be better every day through excellence, compassion and expertise in all we do. - To provide a consultant level service, with a flexible, autonomous approach in an innovative and evolving service. The overriding principle of this service is to provide care as close to home as possible, through integration with all professional stakeholders including mental health, acute, community and primary care services - To provide consultant level clinical input to identified community in-patient beds, and to agreed integrated locality hubs. - To provide clinical leadership in the development of Westhaven, Bridport and Mid Dorset integrated locality hubs, participating in the management of a virtual caseload. - To provide clinical support to the community teams working as part of the integrated locality hubs. This includes the clinical supervision of the doctors and Advanced Nurse Practitioners managing patients as part of the integrated locality hub. - To provide specialist support to local GPs in the management of frail, elderly patients, including, where necessary, domiciliary assessment, step-up admission or clinic review. - To participate in in-patient and hub multidisciplinary team meetings as part of routine clinical work. Alongside this, to participate in regular ward rounds and MDT meetings both on the wards and in the community hub. - To ensure prescribing practice complies with policies and the formulary agreed by the Trust taking into account national and local guidance. The "In Possession Medication Policy" must be adhered to at all times with appropriate risk assessments being carried out and documented as necessary.
