Our new Integrated Community Health team brings together the following aspects of our work, from across the charity (Bromley by Bow Centre) and GP Partnership (Bromley by Bow Health) to create healthy and connected communities:

- Social Prescribing
- Population Health
- Community Development and Outreach
- Welcome Hub - the first interaction many people have with us
Our social prescribing service helps to strengthen personal and community resilience, and reduce health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity. The service aims to increase people’s connectionwith others and their local community and empower them to take control of their own health and wellbeing. We are seeking a Social Prescribing Link Worker who will support the delivery of the Integrated Community Health Team through the provision of one-to-one,person centred support in the Centre’s Welcome Hub.
The role will provide one-to-one-person centred support to service users visiting the Bromley by Bow Centres Welcome Hub, focussing on what matters to the individual and taking a holistic approach, connecting people to local initiatives and services for practicaland emotional support.
The Welcome Hub is often the first interaction someone will have with the Centre, either in person or remotely. The support and information given and the impression created has the potential to reassure and inspire people to connect with services, activitiesand people that can help them achieve so much, both personally and in their own communities.
You may also have experience in the following: Social Worker, Social Prescribing Link Worker, Advise, Counselling, Not For Profit, Charity, Advocacy, Nursing, Advice Worker, Service Coordinator, Project Coordinator, Service Delivery, etc.
