To join a team offering leaving care services for young people aged 16 to 25 including refugees and asylum seekers. To support young people’s transition to adulthood and development into independent living. To provide appropriate advice, assistance and support to enable them to meet their practical, physical and emotional needs. To act as an appropriate role model, working closely with them and supporting them to address their difficulties and achieve their full potential.

MONDAY-FRIDAY 8am
- 4.30pm

MONDAY-FRIDAY 10am-6.30pm

This is on a self employed basis.

**MAIN AREAS OF RESPONSIBILITY**
- To maintain a high quality of care and support which meets the physical, emotional, intellectual, social and cultural needs of children and young people within Assist Cares strategic and operational framework, policies and procedures.
- To provide a caring, supportive and nurturing environment in which young people can feel safe, secure and free from harm.
- To develop positive and trust relationships with young people providing advice, guidance and assistance where appropriate
- To explore the consequences of both acceptable and unacceptable behaviour to enable young people to make reasoned decisions over their actions.
- To support young people in practical matters relating to independent living e.g. (cooking, cleaning, personal hygiene etc.)
- To build positive relationships with young people to enable their engagement with the project to achieve successful outcomes.
- Attend and engage in briefings, de-briefings and team meetings as required.
- Attend regular supervision and Performance Management and Development Scheme meetings with the Project Manager.
- To comply with and implement Assist Cares policies and procedures.
- Undertake training in line with the Assist Cares staff development policies and complete induction and basic training within agreed timescales.
- Empowering young people and facilitating their active involvement in the decision making about their lives and future
- Encouraging the young person to develop links with the community, attend off-site activities and expand their personal social network
- To deal with telephone enquiries and undertake administrative duties as required e.g. (log books, daily information sheets, accident book, incident forms, population returns etc.)
- To develop a responsible attitude with residents with regard to the fabric, furnishings, equipment, supplies and services in/or to the establishment
- The duties and responsibilities listed above describe the post as it is at present. The post holder is expected to accept any reasonable alterations that may from time to time be necessary.

**TEAMWORK**
- Being aware of the aims and objectives of Assist Care and working collaboratively with colleagues to achieve them
- Attending team/ staff meetings and making a positive contribution to them
- Actively contributing to the development of the team
- Being willing to give and receive feedback on performance with colleagues and managers
- Attending and contributing to regular supervision sessions in line with Assist Cares Staff Development Policy

**GENERAL RESPONSIBILITIES**
- Receiving training appropriate to the role and maintaining an up to date training profile
- Responsibility for the accurate maintenance of financial records appropriate to the duties of the post Responsibility for the health, safety and welfare of self and colleagues in accordance with the requirements of the organisational Health and Safety Policies

**SPECIAL CONDITIONS**
- This post requires the holder be flexible and will sometimes mean working outside the normal contracted hours.
- On occasions you may be requested to change your rota at a given notice as per your contract, to ensure the contingencies of the service are covered. This may also include covering an additional sleep-in duty as an emergency measure.
- In accordance with the guidelines on Health and Safety, to accept responsibility for working within these guidelines and reporting any concerns to the Project Manager.
- To undertake such other duties appropriate to the grade of the post and the needs of Assist Care such as cleaning and maintenance in order to develop and maintain service delivery.
