Our South East Norfolk Community Mental Health Team is seeking a motivated Assistant Practitioner to join the existing team.

Our dedicated team covers towns and villages from Wymondham down to Diss, across to Poringland, Long Stratton and over to Harleston. Your base will be Gateway House, Wymondham.

Working across the rural community in South Norfolk our passionate team respect and value the lives of the people we support and work with. We positively engage with a range of companion organisations to ensure the best support and treatment can be given to our service users.

In this vital role, you’ll be a key member of the clinical team, providing high quality care for our service users whilst promoting the principles of Recovery. We’re a forward-thinking, collaborative group who truly value the Assistant Practitioner role.

We are keen to offer opportunities for the successful applicant to develop and enhance their clinical skills - you’ll be encourage to undertake further training, gain additional skills and continue to develop during your career with us.

If you have a Foundation degree in Health and Social care (or equivalent), have experience or a keen interest in working within mental health and are looking for a new challenge in a rewarding community based role, we would love to hear from you

No two days are the same for us; we’re a busy and hardworking Adult Community Mental Health Team and in this role you can expect to be involved in:

- Managing a (capped) caseload working alongside an experienced and supportive team of professionals committed to delivering excellent care.
- Attending multi-disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant Psychologists
- Attending outpatient clinic appointments
- Undertaking face to face visits in the community setting including home visits
- Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
- Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management. you will be supported to develop your skills and knowledge and benefit from regular supervision from colleagues.
