In the 0-19 service, we provide early intervention to empower families to make positive steps to support their own health and wellbeing. We also work in collaboration with our partner agencies such as social care, educational teams and community family hubs to promote the best health and wellbeing outcomes guided by evidence based practices and The Healthy Child Programme (Department of Health 2009). To do this we complete holistic assessments, which includes identification of physical, emotional and social issues to enable us to provide person /family centred support, promoting healthy choices and interventions all the way from the pre-natal stage until the age of 19. Our 0-19 service is an integrated team of Health Visitors, Public Health School Nurses, Community Staff Nurses, Nursery Nurses, School Health Workers and Nursing Associates who work together with families or young people.
We currently have a full and part time STAFF NURSE vacancies in our Newcastle Health Visiting Team. Working in the community can be a challenging environment, where no 2 days are the same and you will be encouraged to develop a strong degree of autonomy as you are supported through the probationary period and induction package. You will need to be mentally resilient, well organised and have a high degree of professionalism for this role as you will be lone working into peoples homes, schools and community venues which can be both a privilege and a challenge. Although based in one locality across Newcastle city, as a community worker, you will be expected to travel across the city in any working day, so a Car driver is essential. Please note that as this a Monday to Friday post you will not get enhanced hours. Accountable for assessing, planning, delivering and evaluating high quality patient care. Provide effective support to both patient and carers throughout their care pathway.
Required to support a team of Health Visitor / Public Health School Nurse /Specialist Community Practitioner, assisting in the delivery of a family centred public health service to a defined population. Working with a multidisciplinary agency approach. To assist in the development of a local health needs profile, supporting and informing service in line with clinical governance framework. The post holder will participate in assessing risk and needs withindividuals, families, groups and communities and then plan implement and evaluate interventions and support which will contribute to health and well-being.
Act as role model, teacher and supervisor for junior staff and students, participating in mentor and preceptorship programmes. To abide by Trust policies and procedures. Act as patient advocate in accordance with Nursing and Midwifery Council (NMC) Code. Work collaboratively as an effective member of MDT.
