Two new senior clinician roles have been created to work specifically within the Access and Assessment function of the newly designed Integrated Community Mental Health Hubs. This is a developing service and part of a pilot phase of the Community Mental Health Transformation project. These roles will contribute to the development of a community system wide ‘one front door’ for the people of Leeds with serious mental illness.
The Access and Assessment senior clinicians will work within the access, triage, and enhanced assessment functions of the service.
You will be enthusiastic and motivated, with a genuine interest in facilitating people getting the right help and support to meet their needs in a timely way.
You will use assess people that access the service using best practice clinical judgements. You will positively engage with service users, family members, friends, and other referrers using a helpful conversations approach. The role requires a calm approach and an ability to maintain professional curiosity into people’s mental wellbeing. The post holder will need to be highly organised, a good communicator. They will possess risk assessment and management skills and develop initial safety plans where needed and offer advice and support regarding management of identified risks.
The post holder will also work closely with call handlers to support with people who may call with high level of clinical need.
You will be required to have an inclusive and trauma informed approach to peoples care who access the service.
The post holder will also have opportunities to work with a variety of stakeholders across various services to further ensure that people are able to access the right support.
The service will operate between the hours Monday - Friday 9am to 5pm. There are opportunities for flexible shift patterns and working.
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. The post holder will need to hold a core registration relevant to the post
Have the minimum of 18months post registration experience of working with adults of all ages with mental health problems.
The post holder will need to have experience of supervising and support other staff and be able to work as part of a wider hub team of professionals including partner organisation to promote partnership working and co-production.
The post holder will need to demonstrate strong levels of skill in relation to risk assessment, helpful conversation calls and referral screening and be able to document these effectively.
Finally the post holder must be able to work in an inclusive, values based and trauma informed approach to peoples care accessing the service.
