HACR Social Worker**

**Grade 7/8 £36,378-£45,576**

**Please note that a newly qualified Social Worker would be appointed on the minimum of Grade 7.**

The Hospital Assessment and Community Review team in Havering is looking for enthusiastic and dynamic Social Workers to come and join our team. The role will primarily support the acute hospitals and also our linked community hospitals across Havering. We operate under a trusted assessor and discharge to assess model, with social workers supporting the very complex hospital discharges. Follow up assessments under the Care Act 2014, are also a part of the teams remit, to establish long term needs post discharge.

Our service runs from 9am-5pm Monday to Friday and 9am-5pm on Saturday with the potential for some Sunday working, we can offer a flexible working pattern and part time or full-time hours.

You will need previous experience in adult social services, with a keen interest in hospital discharges. This area of social work has changed significantly and is in constant flux, so a positive history of managing change within a multidisciplinary setting is desirable.

We are looking for people with confident assessment skills, knowledge of the relevant legislation such as the Care Act and the Mental Capacity Act and an understanding of continuing healthcare process.
