**Support Worker, Wokingham** Full Time 37 Hours £22,572.70 (£11.70 per hour)**

Easthampstead Road, a 24/7 supported service for 3 male customers (no personal care required)

As a Support Worker you will provide high quality person-centred support and care to a wide range of individuals.

For this role you will work as part of a team to support our customers with daily living skills and activities and domestic duties, whilst promoting their independence.

You will also support with, and organise activities, outings and recreation. This role includes responsibility for key working for our customers and working closely with other professionals and family members to ensure health and well-being.

Support consists of encouraging the customers to maintain a clean home, assist with shopping and cooking healthy meals.

Arranging medical appointments and supporting with daily correspondence.

One customer is very active and has daily support to attend activities out in the community. He likes to take a long walk daily.

Shifts are on a rotational pattern with every other weekend off, shifts include mornings, afternoons, evenings and sleeps to cover around 24/7. All staff are required to be able to cover the complete rota shifts.

You will also be driving the Optalis automatic vehicle as required and the customers being supported also have their own vehicles.

This location is close to Wokingham Town Centre and is accessible from the train station and a direct bus route.

This particular service is responsible for actively enhancing customers lives to become valued members of their community, to develop friendships and relationships, have a life that makes sense to the individual.

Encouraging customers to be able to follow their interests and hobbies and have the chance to enjoy new experiences working with a can-do attitude.

Developing daily living skills - thus ensuring individuals develop independence - breaking tasks down into small manageable steps.

Supporting this for all individuals but taking the lead where necessary acting as a key worker.
