Primary Care Lead

We are seeking a Primary Care Lead to provide expert clinical leadership and support the Operational Primary Care Manager in delivering a nurse-led integrated primary care service at our prison sites.
This role involves working closely with colleagues at nearby Kent prisons and occasionally providing cross-cover.

In this role, you'll be the driving force behind our Primary Care service, steering a team of diverse and talented healthcare professionals to deliver care that is not only efficient but deeply centred on the patient.
But that's just the beginning, you will be the architect of new standards in healthcare within our establishment. From developing to implementing and monitoring unique quality improvement initiatives, clinical protocols, and guidelines, your work will directly enhance patient safety and outcomes.
Your role will extend beyond the walls of HMP Swaleside. You will develop and nurture vital relationships with other healthcare providers, prison staff, and external agencies, promoting an integrated approach to healthcare.
Collaborating with the Operational Manager, you will have a significant role in strategically planning our healthcare services, aligning with national standards and local needs. Your visionary approach will identify areas for improvement and innovation, allowing you to lead on initiatives that transform service delivery.

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. Our multidisciplinary teams look after people of all ages and we work in close partnership with other parts of the NHS, local councils and the voluntary sector and through our new provider collaboratives. Our 4,300 members of staff work in many different settings including hospitals, clinics, prisons, secure hospitals, children’s centres, schools and people’s homes.

**Key Responsibilities**:
Provide expert clinical leadership and support to junior staff.
Develop, deliver, clinically audit, and evaluate the Integrated Primary Healthcare Service.
Ensure the implementation of effective patient-centered care planning tools.
Collaborate with multidisciplinary teams to meet and review service needs.
Undertake training to maximize uptake of NHS screening, health promotion, and prevention activities
**Management & Leadership Responsibilities**:
Support the development and delivery of an effective integrated healthcare service team clinical model.
Manage a specialized and complex clinical caseload.
Maintain National Standards for Health Care in Prisons.
Develop strategies to promote and improve health and prevent disease.
Monitor staff performance and compliance with clinical policies and procedures

All applicants must be willing to undertake National Security Vetting in order to work in a Prison Setting. This will be completed as part of the pre-employment checks through Oxleas and the prison vetting team.
