Patient Flow Discharge Tracker

To be first point of call for staff with issues regarding complex discharges. Participate in Plan for Every Patient and Board Rounds on wards identified from Transfer of Care Hub lead and ward teams.
To work autonomously once case load identified, managing workload to objectives agreed from daily MDT.
To ensure referrals and notifications are issued in a timely manner.
To develop a rapport with patients and their families/carers, to encourage their interest and participation in planning for leaving hospital.
Identify issues which may affect the patients discharge home and work with the MDT and other agencies to resolve them.
To act as a central point of contact for discharge planning acting as resource for knowledge of the discharge planning processes, services available and how to access them and contribute to the medically optimised review meetings.
Identify complex patients at Board rounds and ensure all discharge plans actioned.
Once a patient is identified as having complex discharge needs, ensure ‘your discharge home’ letter is given with verbal explanation of discharge process.
Participate in daily Ward Rounds and ensure Plans are completed in full, including PDD
