Band 3 Hub Coordinator

We require enthusiastic and motived individuals to work as part of the Single Transfer of Care Team - Central Hub. The role of a Hub Coordinator is fast-paced, busy, with changing priorities, and we are seeking staff to join this highly valued team. Supporting effective patient flow across all Pathways for admission and discharge, your contribution to the work can be both rewarding and influential as the service continues to develop and evolve.
For all posts there will be a requirement to actively support the administrative processes for referral management, coordination and support.
If you have a passion for supporting patients, enjoy the challenge of problem-solving and using your initiative on a daily basis, whilst liaising with many different levels of staff; and patients’/carers, then we would like to hear from you.
