Starting salary depending on grade:
£33,820 with the opportunity to progress to a maximum of £37,261 per annum (Occupational Therapist)

£38,296 with the opportunity to progress to a maximum of £42,503 per annum (Senior Occupational Therapist)

Plus relocation package, £1,000 welcome payment & lease car* terms apply

Hours: Part time and 37 hours per week

Vacancies:
Adult Disability Team East Hertfordshire - Stevenage
- North Herts & Stevenage / East Herts

Provides support to adults with a learning or physical disability and we cover the East and North Herts area of the county. We promote personalisation through our new assessment process focusing on the wellbeing of the individual. Through care planning westrive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.

Older People Team East and North Hertfordshire - Stevenage
- Welwyn & Hatfield / East Herts / Broxbourne / North Herts

Our locality teams are diverse teams made up of Social Workers, Occupational Therapists and Community Care Officers. These teams focus on extended involvement of complex cases and includes general assessment and care planning as well working with our Safeguardingteam to ensure risks to vulnerable people are minimised.
- Enablement Team

Provides support to older people and we promote personalisation through our assessment process, focusing on the wellbeing of the individual. The Enablement Occupational Therapists work alongside the care providers to deliver enabling programmes. Their inputis time-limited, with intervention generally up to 4 weeks. We support people in the community and on discharge pathways from Lister and Princess Alexandra Hospital.
- Review Team

Provides support to adults with a diagnosis of dementia and older adults by reviewing those in a residential or nursing home setting. This can include systematic reviews, as well as reviews due to a change in circumstance or a safeguarding concern. The teamworks closely with our commissioners to complete targeted reviews when we have potential concerns with care providers.

Adult Disability Team West - Hemel Hempstead
- St Albans / Review Team

Provides support to adults with a learning or physical disability. We promote personalisation through our strengths-based approach focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice, and controlby offering personal budgets to enable people to direct their own support. You will need to be a confident individual, capable of working independently with complex cases, enabling people to live independently in the community. You will conduct holistic assessments,including moving and handling, minor and major adaptations, social care needs and will plan and care manage cases to achieve desired outcomes. You will work alongside Social Workers, Community Care Officers and Community Learning Disability Nurses to achievethe best outcomes for the individuals you work with.

Older People Team West - Hemel Hempstead
- Dacorum

Your role will primarily be to carryout comprehensive assessments to enable independence and Moving & Handling assessments for elderly people who are struggling to manage activities of daily living in their homes or in placement. You will be working to respondpreventatively to referrals for social care and support and will use occupational therapy interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence and to enable/supportcustomers to achieve their desired outcomes.
