To monitor and proactively track patients on the Patient Tracking List (PTL) and maintain the trusts RTT tracking system using advanced knowledge and understanding of Patient Centre, Evolve and patient pathways. To monitor and provide guidance and training, to clinical and administrative staff on the use of RTT status codes, ensuring their accurate use by all staff members. To present information regarding RTT processes to all levels of staff, clinical and administrative, adapting complex RTT data as appropriate for the target audience. To escalate problems with capacity with relevant staff and work with colleagues to implement solutions to capacity issues.
To use multiple clinical information systems and databases to understand and coordinate patient pathways. To proactively identify and support services in the resolution of pathway delays. Using specialist knowledge, to update RTT pathways where information is identified that has not been included on the pathway which may affect clock start and stop dates. Have autonomy to investigate and validate patient pathways in liaison with services, using own initiative to ensure breaches are avoided.
This would include productively challenging the progress of patient pathways with relevant staff members. To be responsible for ensuring that patient pathways are correctly linked and reflect the patient journey. To highlight bottlenecks with the patient pathway, communicating regularly with service management and where possible suggesting and implementing solutions. Any other duties appropriate to the post-holders grade.
Staff may be required to undertake any other duties at the request of the line manager, including project work. To ensure that incoming referrals are accompanied by accurate and timely pathway information and that this is being recorded correctly. To deputise for the RTT and Referrals Team Leader, in relation to data reports, if required. To follow existing policies and procedures, as well as proposing new changes to practice if deemed appropriate.
