**Overview**

As part of a multi-disciplinary, multi-agency approach, you will be confident to work with people at all levels, forging working relationships. You will manage a caseload and will ensure that the service delivers the best possible experience for people accessing support, their careers and families.

**Duties**

**1.**To provide trauma informed peer support to service users who have a range of mental health conditions and/or learning disabilities.

**2.**To manage a caseload of individuals and provide 1:1 support including, but not limited to:
a. Working in a solutions-focus manner to assist clients to overcome a range of barriers so that they are able to access clinical and non-clinical support.

b. Assessment and co-production of appropriate action plans to enable clients to improve their mental health.

c. Maintaining regular supportive contact with each person to assist them to achieve their goals.

d. Identifying a holistic package of support to assist the development of each individual.

e. Plan and deliver peer support for individuals and groups within the limits of the peer support framework.

To work in communities to promote the service and recruit appropriate clients.

**4.**To be responsible for data processing including inputting information onto databases and creating reports.

**5.**To assist with overall service marketing and promotion activities, including supporting mental wellbeing and health events and publicity campaigns as appropriate.

**6.**To establish and maintain strong relationships within the service and with VCSE delivery partners.

**7.**To develop and maintain excellent working links/partnerships with external agencies to support delivery and assist people to reach their goals.

To conduct personal, venue and activity risk assessments as required and to ensure Health and Safety and Safeguarding procedures are always adhered to.

**9.**To be an ambassador for the VCSE and represent the service at a number of different meetings, forums and events, as appropriate.

**10.**To operate within the policies and procedures of The Cornforth Partnership and the project, including confidentiality, equality and diversity, information governance and data protection.

**11.**To understand own limitations and boundaries.

**12.**To demonstrate a commitment to continued personal development, maintaining an up-to-date knowledge of developments across the field, legislation and practice relevant to the service.

**Requirements**:

- Relevant vocational qualification - level 3 or equivalent (Health & Social Care, Health Trainer, counselling etc). Essential
- Numeracy, Literacy and ITQ Level 2 or equivalent. Essential
- A willingness to undertake mandatory and role specific training within a specified timescale. Essential
- Good overall sector based training covering a range of motivational and behavioural change techniques along with health and wellbeing interventions. Desirable.

**Job Types**: Full-time, Part-time

Pay: £24,000.00-£24,420.00 per year
