Lead Midwife for Governance and Safety

We have an exciting opportunity in our Maternity Department to to support the Head of Midwifery to embed an open and transparent culture and governance framework that facilitates the delivery of safe, high-quality care.
This will include a Care Group-wide approach to Quality and Safety, Risk Management, Patient Experience and compliance and will include a specific remit to ensure robust processes are in place to meet the service’s requirements for providing excellence in the delivery of maternity care.
They will work closely with the Head of Midwifery, Senior Leadership Team / and Nursing Senior Teams to ensure the smooth and effective management of the Maternity and Gynaecology Service.
The post holder will support and work closely with the Heads of Midwifery for Women’s Health in ensuring robust processes are in place to meet the service’s requirements for providing excellence in the delivery of maternity care.
The post holder will have responsibility for oversight of the Maternity and gynaecology risk, incidents, serious incidents and learning from harm.
- To provide oversight to ensure that the grading of incidents is accurate and that the level of investigation is commensurate with the harm caused and likelihood of recurrence
- To interpret data and produce analytical reports of investigations identifying themes and lessons learnt
- To be actively involved in SI investigations.
- To contribute to the investigation and analysis of incidents within the Care Group
- To review Serious Incident investigation performance to ensure the timely submission of final investigation reports to the Director Review Group
- Provide expertise in the assessment of risk
- Maintain oversight of the Risk Register, ensuring appropriate escalation of new risks and that mitigations and controls are effective in risk reduction
- Ensure the Governance objectives align with the Trust’s priorities
- Be responsible for the capture and collation of data for maternity statistics, dashboard and other KPIs.
- Embed the outcome measures as defined by the CQC within the service
- To maintain an overview of all ongoing investigations and to assist with progress when required
- To assist with the allocation of initial management reviews (IMRs) within the team when required
- Ensure timely escalation of immediate concerns

- Demonstrate an organisational approach to the development of maternity services
- To provide senior expertise to investigators of incidents to ensure appropriate lessons learned are identified and action taken is documented
- To oversee monitoring of action plans to improve adverse trends and prevent recurrence and ensure that evidence of completion is robust
- To ensure all incident queries and investigation queries are dealt with in a timely manner.
- To ensure that the MSE Serious Incident Policy is fully embedded in the Care Group.
- To support the development of effective actions to reduce risk and proactively monitor implementation
- Support the development, formulation and implementation of unit guidelines, policies and protocols
- In conjunction with the Director of Midwifery and Heads of Midwifery/Nursing Lead on the unit’s governance agenda and liaise closely with the Trust Associate Directors for Patient Safety and Risk & Compliance.
- Have knowledge of the Trust infection control policies and ensure staff adhere to them.
