Our Healthy Child Programme Service 0-19 incorporates Health Visiting, School Nursing and Teenage Pregnancy Services. It is an exciting time to join our service and be part of developing and shaping services for babies, children, young people and their families.

An exciting opportunity has arisen for a dynamic, motivated and compassionate qualified SCPHN to lead a newly formed team. This is a new role and part of the establishment of new team which reflects the need to be increase capacity to provide care and support babies, children and young people with additional needs, disability and/or SEN.

You will be part of wider team around the child provision which is well established for 0-5 and has an international reputation, recently having presented at good practice conference in Europe.

You will need to demonstrate your ability to lead teams through times of change, engage staff and children & families to develop the provision. You will need to work with the professional/deputy professional leads to develop pathways of care. You will have an excellent record of working in partnership with other agencies and services

The post holder will have day to day operational responsibility & line management for a team of nurses, NNEB’s and support worker. The post holder will lead on the 0-19 Healthy Child Programme 0-19 pathway for babies, children & young people with disabilities and additional needs. This includes providing outreach support to the School Nursing and Health Visiting locality teams. The team will provide the HCP 0-19 offer to MLD & SLD schools and specialist 0-5 provisions. The team will work with a wider multi-disciplinary team and external services to provide a team around the child & family approach to service provision, ensuring an all year around service is offered.

Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which will commence in 2021

We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

Key Responsibilities

Leadership
- To line manage the nursing team, providing advice and support to the team, provide caseload supervision, manage sickness/absences, annual leave, and study leave in accordance with the Trust policy, participate in the recruitment and selection of staff.
- To undertake individual performance reviews and formulate personal development plans with those staff. Provide learning opportunities, induction and preceptorship for all learners.
- To be aware of budgetary constraints ensuring that the team works within their allocated budget, identifying future cost pressures. To ensure that resources are managed and used effectively.
- To develop, implement and evaluate new and existing policies ensuring that practice is evidence-based.
- Maintain clinical governance processes, e.g., identification & management of risks and incidents, patient complaints.
- To act up in the absence of the deputy professional lead or professional lead and/or represent the service at operational or strategic meetings as directed.

Clinical Skills
- To always practice within NMC standards and the Code of Conduct.
- To work in a multidisciplinary setting and team to promote the needs of children with special needs, prioritising workload and developing effective partnership working with others and play a leading role in the development of new services.
- To plan, develop, implement and evaluate programmes of care for this client group.
- To co-ordinate the health needs assessment of children under the care of the team, participating in the multi-disciplinary assessment of babies, children & young people.
- To provide clinical advice and support, on aspects of care for babies, children & young people with disabilities and additional needs to families and colleagues within the locality teams, integrated team, and other agencies.
- Take the lead professional/key worker role for babies, children & young people when appropriate and work as part of multi-agency teams to support those on child protection, child in need and early help plans or those who are looked after.

- To advocate for families within the multidisciplinary team and make referrals as appropriate.
- To work closely with the Special Care Baby Unit, Children’s Ward and Maternity Unit, special schools and education
