**JOB SUMMARY**:
Housing sits at the heart of our organisation. We believe that everyone deserves a safe, secure, affordable home. Providing care and support to those who need it is an important part of what we do. We work with people experiencing homelessness and who oftenhave other support needs. Our services include hostels, young peoples' accommodation services and support delivered in the community; where we provide tailored support that prepares our customers to achieve independence and re-integrate within the community.
**RESPONSABILTIES**:

- Working as part of a team working to support individuals (with low-medium support needs) to maintain their tenancy, avoid eviction and improve long-term positive outcomes. Carrying out support planning and risk assessments for customers with low-mediumsupport needs.
- To support the delivery of the commitment to equality of opportunity through actively promoting equality and diversity in the workplace and in the service delivered. To be aware of current national and local policy and good practice requirements in relationto older adults, dementia and disabled people and to contribute to its implementation in the service delivery at all times.
- Supporting the customer with property viewings; sign-up to the OHG tenancy and to sustain the tenancy. Collaborating with relevant community agencies to ensure customers receive appropriate advice and support.
- To facilitate and enable service users to identify and access social, training and employment opportunities in their local community to maximise social inclusion and physical social and economic independence. This may include on occasion driving the servicevehicle when required to facilitate group activities, where service users are unable to use any other form of transport.

**EXPERIENCE**:

- Be able to demonstrate specific knowledge of a Support Setting. Have care Homelessness and Prevention experience.
- Experience of managing cases, in accordance with agreed procedures and completing assessments of need and risk.
- Experience of planning, prioritising and evaluating activities and own workload effectively.
- Experience of demonstrating a comprehensive understanding of safeguarding and an ability to promote the wellbeing and welfare.

**Also, will be required to go through an enhanced DBS check.**
