The service is positioned at the heart of the Trust's community services, providing acute and reablement care at a fast pace. The post holder will be a critical key member of the vast multidisciplinary team, supporting the multicultural Coventry community and wider system admission avoidance strategies.

These new roles are designed to be dynamic and exciting. You will be responsible for supporting elements of clinical triage, including case-load management.

The role will include data collection and analysis and the scoping and reporting of capacity and demand at regular intervals. The role will include working with registered clinicians to facilitate the coordination and allocation of work to both registered and unregistered staffing groups within the service.

You will support patient movement across wider system partners to ensure patient centred care is delivered across services within agreed timescales. You will keep IT systems updated with regard to patient data, as well as prepare data driven reports as required to support system transparency of service capacity. You will support and lead on projects relating to patient flow looking in detail at capacity and demand models.
