**Personal Health Commissioning Who are we?**

We are a team within NHS Dorset ICB that undertakes assessments, reviews and sources care for individuals eligible for Continuing Healthcare, Funded Nursing Care, Joint Funding, Section 117 Aftercare, Complex Rehabilitation and Children and Young Peoples Continuing Care.

We pride ourselves on taking a person centred approach to establishing how the individual needs of patients we support are best met.
