**Job Overview**

**Duties**
- Assist patients with daily living activities, including personal hygiene, mobility, and meal preparation.
- Implement care plans as directed by healthcare professionals to support rehabilitation goals.
- Communicate effectively with patients to encourage participation in their rehabilitation process.
- Document patient progress and report any changes in condition to the supervising staff.
- Maintain a clean and safe environment for patients, ensuring all equipment is properly used and stored.
- Drive patients to appointments or therapy sessions as required.
- Collaborate with other healthcare team members to provide comprehensive care.
- Engage in ongoing training and development to enhance skills related to home care and rehabilitation practices.

**Experience**
- Previous experience in a care home or home care setting is preferred but not essential.
- Familiarity with IT systems for documentation and communication purposes is advantageous.
- Proficiency in English is required for effective communication with patients and staff.
- A compassionate nature, coupled with strong interpersonal skills, is essential for success in this role.

**Job Types**: Full-time, Part-time

Pay: £11.60-£12.60 per year
