This job is supporting secretarial services across 2 GP surgeries and our care bed/intermediate care service. The applicant can work from one site supporting the others remotely.

East Park Medical Centre serves a patient population of approximately 8500 over two sites, one in East Park and the other Halton. Guiseley and Yeadon Medical Practice is situated in West Leeds and serves a patient population of nearly 12000. It offers services out of Yeadon Health Centre and its branch surgery in Guiseley.

East Park Community & Care Bed Service is currently responsible for looking after Burmantoff, Harehills and Richmond Hill PCN’s care home beds and 4 Community Care Bed facilities for 160 Assessment and Rehabilitation (ARB) beds.

The ARB service oversees the care of medically fit patients who are discharged from hospital following an acute admission but are not yet able to manage/ return to their homes. During their admission to the community bed facilities a multidisciplinary approach to their care focuses on rehabilitation and maximising independence. The EPCCB service is responsible for their medical care alongside input from consultant geriatricians. The Care Home part of the service is responsible for delivering the PCN Care Home enhanced service

**Specific Duties and Responsibilities of the Post**
- Process patient referrals accurately, ensuring all required fields/data have been entered to ensure referral acceptance.
- Liaise with GPs with any queries or missing data to ensure referrals are dealt within the set timeframe.
- Undertake typing duties using electronic digital system as required
- Maintenance of a Practice directory for services available to patients, to include Consultants, Chiropodists, and other services available outside the practice
- Training of new secretarial staff members in line with departmental/practice policies and procedures, ensuring ongoing reviews
- Electronic system maintenance including review of outstanding referrals and rejections
- Receive and initiate telephone calls and deal with appropriate queries
- Assist with meetings and minute taking as required
- Answering incoming telephone calls, ensuring calls are documented and redirected accordingly.
- Provide advice to less experience colleagues and maintain knowledge on referral processes and procedures, relaying to other Practice staff where necessary.
- Provide medical information to patients using practice guidelines
- Ensure up-to-date maintenance of both computerised and manual filing systems (i.e., patient notes) in an accurate and secure manner.
- Always work safely in accordance with Legislative requirements and Practice Policy and Procedures.
- Work across multi sites will be required although this can be done remotely most of the time
- Keep up to date with current training
