Under the direction of the Outpatients Matron, the post holder is to be responsible for the effective delivery of care given to patients within the Outpatients Department.
- The post holder is responsible for the day to day operational and clinical requirements of the clinics in the designated area.
- To work alongside all members of the Outpatients multi-disciplinary team to ensure the clinics are run safely and effectively.
- To develop patient care and service delivery in conjunction with the OPD senior sister and Matron.
- To anticipate the care needs of patients and actively manage the patient’s journey by pro-actively participating in the care as part of the multidisciplinary team.
- To assist the Outpatients senior sister and Matron to deliver the Outpatients strategy.
- To deliver care in accordance with Nursing & Midwifery Council Code of Conduct & Trust policies

We are looking for a junior sister to work within our Outpatients Department at West Middlesex University hospital. A professional, caring, self-motivated and confidence to support the service, with experience of caring for patients with varied surgical or medical and other varied specialist conditions.
The post holder will be responsible for the day to day operational and clinical requirements of the clinics in the designated area.
To deliver care in accordance with Nursing & Midwifery Council Code of Conduct & Trust policies

We are a very busy department that functions between Monday to Friday, 0830 to 1700, along with staggered time in the department as the clinic sometimes overruns.
An essential requirement to this post is the ability to communicate effectively within a multi-disciplinary team, and with a diverse local population. We aim to continually improve the patient experience throughout our Out Patients Department.

Roles and responsibilities
**COMMUNICATION**:

- Maintain effective, accurate communication and dissemination of information to all staff, patients and visitors.
- Give information to patients; including helping resolve their queries with colleagues and other health care professionals as appropriate.
- To communicate distressing information to patients and provide the necessary support as required, involving the multi-disciplinary team when necessary.
- To ensure information is communicated to patients in manner that encourages and promotes understanding, taking into consideration any difficulties with language or hearing.
- Give information and advice to patients regarding health promotion.
- Communicate with other members of the multi-disciplinary team to ensure patients receive prompt, effective care and treatment.
- Put in measures to anticipate the information needs of others.
- Ensures patients are kept informed about their treatment and the waiting times in clinics.
- Ensure effective communication is established within the nursing team by holding regular team meetings and recording discussions and following up on actions agreed.
- Maintain accurate documentation, both paper and electronic, in order to ensure timely and effective communication.
- Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a responsive and inclusive manner, focusing on improvement and ways to move forward.
- Ensure all communication is presented appropriately to the different recipients, according to levels of understanding, type of communication being imparted and possible barriers such as language, culture, understanding or physical or mental health conditions.
