This is a realistic fellowship, as we feel there are enough training opportunities at PCH to serve the training needs of a fellow with 2-4 laparoscopic resections a week. The goal of the fellowship would be to attain competence at laparoscopic colonic resections and introduction to the principles of laparoscopic TME. In addition, the fellow could join in other open cases, redo resections, emergency colorectal surgery joint cases as required by training needs.

We hope the fellow would be JAG accredited, thereby allowing some independent endoscopy lists. There are opportunities to keep your hand in with lap cholecystectomies, herniae etc (training registrars will have first priority for non-colorectal cases)

The fellow would have an administrative role in the management of the colorectal unit and the junior staff, as well as rota organisation within the colorectal unit. Additional responsibilities would include database management and overseeing the local ERAS program.

We would anticipate at one publication to be generated for the 6 months, this is a key requirement of the job.

The fellowship is designed for yr 5/6 trainees, preferably having passed their CCT. We would be interesting in pro-active individuals with some management/leadership skills.

The fellow are required to interact with RCSEng to provide feedback and other details for the fellowship

Teaching

All members of the department are committed to high quality teaching of junior medical staff, both theoretical and practical, taking account of the training requirements and experience and ability of the individuals.

The Department also has responsibility for clinical students from Cambridge on regular attachment. Elective students from overseas are also encouraged.

Clinical Audit

Clinical audit is an increasingly important activity. We have monthly morbidity and mortality meetings where the previous month’s problems are discussed, as well as a 4-monthly review of cases referred to NCEPOD, the SAPOD (Surgical Audit of Peri-Operative Deaths) Meeting. We have a surgical computer system that enables rapid production of data for audit as well as producing discharge summaries. There is an active and helpful Clinical Audit department, which assists clinicians in carrying out their audit projects.

The colorectal unit at Peterborough City Hospital is a busy unit that serves a population of 350,000 and deals with roughly 225 colorectal cancers per annum. The majority of colorectal resections are conducted laparoscopically. PCH is a new hospital that became operational in November 2010. There are 650 beds, 18 operating theatres, a combined ITU/HDU. There is good equipment to facilitate major laparoscopic resections. Energy devices available are Harmonic ScalpelTM, EnsealTMand LigasureTM.

**Colorectal Surgery**: In addition to laparoscopic surgery, for colorectal cancers we perform APERs, ELAPE and reconstruction along with plastic surgeons. Benign colorectal work includes IBD, pelvic floor, THD, endometriosis and complicated diverticular disease. TAMIS is provided for. TATME is being initiated. We also offer IPAA surgery to Peterborough.

**Endoscopy**: The endoscopy unit has excellent facilities and is a bowel cancer screening centre. There are 5 endoscopy rooms with Olympus scopes, Endobase software that allows video recording and good facilities for difficult polypectomy and EMR. Anorectal ultrasound and anorectal physiology testing is also available. Opportunities are available for independent scope lists (if JAG certified) or for colonoscopy training lists if not yet JAG certified. Our endoscopy unit is extremely trainee friendly; all our fellows have attained JAG certification and gone to have their own lists.

Several of the surgeons have undergone laparoscopic colorectal fellowship training at reputable units, in Cleveland, USA; Colchester, UK; Auckland, NZ and Swansea, Wales. The consultant body is keenly interested in teaching and training. The recent development is the merger of Peterborough City Hospital and Hinchingbrooke Hospital into North West Anglia NHS Foundation Trust. The two surgical departments work separately but there may be a requirement to work across both hospitals as the merger evolves.
