**CLINICAL LEAD**

Seacroft Green Care Centre are looking for an Clinical Lead to join us ion our day team - Seacroft Green Care Centre is a luxury purpose built care home in Seacroft, Leeds.

All Clinical Leads must hold an active nurse registration with the NMC and have a minimum of three years experience within a nursing home setting.

**Purpose of the role**

To provide leadership on all issues relating to clinical excellence and coordinating staff to ensure that effective nursing and care services are provided, quality standards are met and maintained, clinical outcomes are recorded and measured. To develop and maintain an environment that supports staff in developing clinical skills and capabilities throughout the service.

**Main Duties and Responsibilities**
- Responsible for the health and social care needs of all residents with nursing needs within the Home
- Systematically evaluating the quality and effectiveness of nursing and care practices in order to improve the services and outcomes for residents
- Undertaking comprehensive assessment of new referrals and carrying out regular reassessment post admission and ensuring that all information is obtained from relevant agencies
- Ensuring that evidence based, person centred care is central to service provision
- Lead in maintaining and evaluating the electronic care plan systems, ensuring that feedback is provided to management and staff
- Carry out regular clinical audits, draw up and monitor action plans
- Ensure the availability, operation and servicing of clinical equipment and supplies
- To lead on the achievement of the gold standard framework programme, and ensure that the service retains accreditation thereafter
- Working with the Registered Manager to undertake annual appraisals and regular supervision of all clinical staff ensuring that individual personal development plans are in place and evaluate staff competency by direct observation
- Promote research, and integrate evidence based identification of best practice in the delivery of nursing and care services
- To be involved in the preparation of the weekly management report and ensure accurate contemporaneous documentation
- To establish a good working relationship with service user’s families and advocates, and hold regular support group meetings
- Mediate with residents and relatives to resolve concerns related to care services and ensuring service user satisfaction
- Work in collaboration with other professionals, relatives and interested parties to enhance the quality of care delivered
- To take responsibility for the Home during periods of absence of the Registered Manager
