Royal Trinity Hospice’s community catchment area spans across south east, south west and north west London integrated care systems and the team support patients with identified specialist palliative care needs wherever they call home.
We have an exciting opportunity for an exceptional individual to join our busy community service as a Palliative Care Assistant (PCA). We are looking for a kind and caring individual who is committed to delivering high quality person-centred care and support to patients approaching the end of life and those close to them.
The PCAs at Trinity make a vital contribution to the ongoing assessment and specialist palliative care support delivered to patients on the specialist community nursing caseloads and are the ‘eyes and ears’ of our service. The support is delivered either virtually or in person. Visits are carried out independently or jointly with a registered professional, and care is always provided under the guidance and supervision of the registered professional responsible for the patient

It is an exciting time at Trinity as we begin launching our new community services, which have been designed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.
Responsibilities of a Palliative Care Assistant
Use listening and observational skills to understand the needs of patients, families, and carers
Communicate effectively with patients, their families, carers, using a variety of methods (e.g., face to face, telephone, or video) to build trusting professional relationships
Support clinical and non-clinical colleagues by communicating observations of patients and other situations clearly and concisely.
Develop excellent working relationships with the different members of the team offering community services
Skills and experience required
Understands the challenges of working with people with life-limiting illnesses and those important to them
Able to undertakes a range of routine assessment and interventions in a community setting but willing to carry out new interventions following appropriate training.
Has completed the Care Certificate or be willing to complete within 6 months of appointment
Understands the importance of multidisciplinary team working and discussion.
Able to use electronic databases and willingness to learn to use those relevant to the role
Able to create accurate records within the electronic patient record system in line with the Hospice’s guidelines
Able to identify and action with support any learning needs or gaps in knowledge and actively engage in learning and development opportunities
