Working hours - 37.5 hours per week "The Countess of Chester are looking for a dynamic, highly motivated, Lead Professional for Complex Care, to join our integrated adult and children's Safeguarding and Complex Care Team. As a Senior Practitioner, you will have a key role in shaping our complex care agenda and work closely with the wider Integrated Specialist Team to ensure appropriate quality assurance mechanisms, are in place for patient's with complex care needs. You will provide highly specialist advice and clinical support to staff employed by the Trust, ensuring there is appropriate assessment, planning and implementation of plans of care for patients with complex care needs. The Trust has defined the cohort of Complex Care patients as those patients who have; a Learning Disability and / or Autism, Dementia, a Delirium or suffer with mental health difficulties. You will hold a professional registration (registered nurse/AHP/Social worker). In return, you will be rewarded with a supportive environment with opportunities for professional development and working creatively within an experienced integrated team" Main duties of the job Provide specialist advice and guidance for clinical teams throughout the Trust on effective assessment, planning, implementation and evaluation of patient care according to changing health care needs, paying particular attention to Mental Capacity, Dementia and Learning Disability / Autism concerns, ensuring that patients and their relatives/carers are involved in all aspects of care To promote good practice and effective communication, within the organisation and also with external stakeholders on all matters relating to the complex care agenda. To promote and ensure that the Mental Capacity Act is an integral part of the Trust business and that key staff are aware of the legislation and practice guidance that supports it as well as enabling ALL patients to be supported in accessing a high standard of care by providing specialist advice on reasonable adjustments To provide expert advice and leadership across the organisation - acute and community and to ensure that the safeguarding principles are embedded within care delivery across the organisation. The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital. The Trust provides services in all of the surgical and medical specialties common to a District General Hospital with some specialist services. The Trust serves the whole of the local population currently in excess of 250,000 residents which geographically covers populations in England and North Wales. The Trust strives to continuously improve and sustain the quality of its services for patients, providing care to more than 445,000 patients per year, with almost 4,000 staff working together putting quality and patient safety, at the heart of everything we do. The organisation provides a wide range of services offering health care to the whole spectrum of the population from birth to the end of life.
