Waking Night Support Worker

Glassmoon Services provides 24 hour registered supported living and complex outreach services. Supporting people with learning disabilities, complex health needs, mental health, autism spectrum conditions and behaviours that may challenge. We offer real choice and independence for the people we support.

**Support Worker - Purpose of my role**

To support and enable People We Support to live their best life, whilst delivering person-centered care, unique to each individual, aligned with our person led support delivery model

**Care at the core**
- provide the best quality care in accordance with my learning, best practice and legislative requirements, reflecting our policies, procedures and agreed standards to supports Glassmoon Services purpose
- support and enable People We Support to engage with their interests and maintain skills, whilst delivering person-centred care unique to each individual
- ensure People We Support are at the heart of my care delivery and their wishes and preferences enhance their wellbeing
- Actively engage with People We Support to develop and maintain their relationships and connections within their local community
- Complete and maintain Support Plans and other information to support People We support and how the support is delivered
- informing the Team Leader/Senior Support worker of any changes I observe in the needs of People We Support
- promote and safeguarding the welfare of those individuals I support
- if desired by the Person We Support, maintain and develop relationships with family, friends and other people important in their life

- be a role model, inspire and support all my colleagues

**Recognising the day-to-day reality**
- contribute to the efficient running of the service and support my colleagues
- work in collaboration with members of the multi-disciplinary teams to maximise opportunities for People We Supports

- keep my knowledge and understanding of the Fundamental Standards refreshed alongside the regulatory framework that governs the service, including the role of CQC and their requirements
- understand and follow all policies and procedures relevant to my role
- understand how my role purpose supports the purpose of the service and Glassmoon Services
