Dorset Healthcare are looking for Senior Community Therapy Assistant to join our supportive team based here in East Dorset Locality
The multi-disciplinary team work, with adult patients in their own homes or care homes to promote independence and improve quality of life by supporting them to remain at home. Therefore, being a car driver is essential for this role. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence.
The Senior Community Rehab Assistant Therapy role will be required to provide clinical follow-up support to people in their own homes following crisis intervention from the Urgent Community Response, carry out holistic assessments, formulate care plans and monitor progress against set goals, under the indirect supervision of a registered therapist.
This role is a part
- time post 37.5hrs per week. Hours covered by the service are 08:00-20:00 7 days per week with weekend enhanced pay
Providing nursing and rehab-based care to people in their own home.
Completing autonomous assessments of nursing, mobility, equipment and function of patients following triage from a registered clinician.
Provide clinical follow-up support to people in their own homes following crisis intervention from the Urgent Community Response service. The support will be provided through ongoing assessment, intervention and reviews with aim to avoid unnecessary hospital admission and prevent avoidable deterioration.
Providing advice and programs of rehabilitation for other support staff to carry out.
Provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy and dignity.
Support the team to respond to patients in crisis to avoid hospital admission and to support hospital discharge. Work alongside other support staff to ensure the patients care needs are met in order to remain at home.
