This is an exciting opportunity to join the expanding Wood Green and Croydon Branch as a Care Coordinator/CM2000 Officer You will need to be a confident care coordinator who is ready to hit the ground running.

**The Care Coordinator is responsible for**:
- Planning and assisting the Manager and Director of Care in the delivery of all required care services within their given area.
- Maintaining effective communication between the local office, local authority, private customers and external agencies.
- Ensuring continuity of care provision through effective contingency plans.
- Being an integral part of the Branch office team.
- Experience in Staffplan & CM2000 is essential.
- Ability to work under pressure with or without supervision & according to business demand.
- Be there for emergencies to accept packages and assess immediately, according to business demand & business development.
- Contacting staff that did not login or logout correctly, documenting and setting up an action plan in accordance with Individualised Care’s policies and procedures, to prevent future occurrence.
- Identify staff that have not stayed the full duration of their working hour when logged in, investigate the reason, document, and report it.
- Send weekly report of CM2000 to management.
- Generating and updating care plans of service users and referring it to the manager for consideration.
- You will also support and assisting in any other administrative and recruitment duties, when required.
- One 2 one supervisions - highlighted through spot checks.

**Required Experience and Qualification**:
- Must be computer literate
- Essential
- Knowledge and experienced of using CM2000 and Staffplan
- Must have excellent Microsoft Excel skills

- Must be competent in using Microsoft Word

- Must have clear written and verbal communication skills
- Essential.
- Must be detail oriented

- Possess good organisational skills.
- Must be hard working and eager to work and learn.
- Must be a driver, with a valid UK driving license

- 7 days a week and 5 days on the second week - Manager's discretion.
