We are currently seeking a dedicated Harm Reduction Practitioner to join our dynamic team and contribute to our mission of providing compassionate care and support.
Ward 5, located at the Newsam Centre in Leeds, is a 17 bedded locked complex rehabilitation service that delivers rehabilitation and recovery to male service users with enduring mental illness. The service provides a holistic, psychologically informed approach to support service users in developing their life skills, independence and hope for the future in a way that is meaningful to them, in the least restrictive way possible, with a focus on individual strengths.
The job role will include providing specialist addiction treatment, including psychosocial and pharmacological interventions, harm reduction, & risk management to service users with co-existing mental health needs. The Ward 5 Team is trauma-informed and offers a formulation driven, psychological model.
**As a Harm Reduction Practitioner, you will**:
Provide specialised treatment and support to service users with complex mental health and substance misuse needs.
Deliver expert psychosocial interventions and offer harm reduction advice tailored to individual needs.
Develop and implement group work programs designed to address the unique requirements of designated service users.
Collaborate closely with the Multidisciplinary Team, other teams, families, and carers to assess addiction issues and contribute to individual care plans, clinical risk assessments, and safety plans.
Offer specialist advice and supervision to the wider multidisciplinary team regarding addiction.
Work collaboratively with operational, clinical, and professional leads to identify and address addiction training needs, developing and delivering workplace learning and educational activities.
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. The Harm Reduction Practitioner provides treatment and support to service users with both complex mental health and substance misuse needs, who may also present with co-morbid complex psychological and physical health needs.
Deliver specialist psychosocial interventions and harm reduction advice. The post holder will also develop and deliver a programme of group work that is tailored to meet the specific needs of the designated service users.
Work closely with the wider Multidisciplinary Team, other teams, families and carers, the Harm Reduction Practitioner will assess specific addiction issues and contribute to individual care plans, clinical risk assessments and safety plans.
The post holder will provide specialist advice and supervision to the wider multidisciplinary team in relation to addiction.
