An opportunity has arisen for a passionate and self-motivated individual to join our team as a **Male Outreach Support Worker **in **Wrexham.**

**JOB DESCRIPTIONHours**:22.5 hrs per week

**Salary**: £21,507.75per annum

**Holidays**: 25 days per year plus 8 public holidays (pro rata)

**Based at**: Adferiad Wrexham outreach / floating support

**Responsible to**:Project Manager & County Manager

**Job Purpose**:
To improve the lives of people with mental health needs by providing one-to-one support to equip them with the skills, knowledge and coping strategies to ensure their own well-being.

Under the direction of the Manager, the post holder will work as part of a team, support clients and contribute to the delivery of the floating support project, in line with Adferiad’s principles of empowerment, self-management and recovery, and to ensure service compliance with internal and external quality standards, and regulatory requirements.

**About this job**:
This service has been developed in partnership with Flintshire Supporting People and the Housing Options Team in Flintshire Council, to provide floating support for adults who have a serious mental illness and who need support to help with their recovery, maintain tenancies or source accommodation.

The aim of the staff team, therefore, will be to encourage and enable clients to reach a greater level of independence over a reasonable period, and then staff are to facilitate / ensure individuals are more independent in the community.

**Main Duties and Responsibilities**:

- Receiving and dealing with referrals from the Supporting People pathway and other agencies including homelessness services, as well as self-referrals with priority given to those who do not meet the eligibility criteria for support from Wrexham Council, Mental Health services or statutory mental health services.
- Identifying and meeting the needs of individuals for a time limited period in accordance with their housing and related support needs.
- Assisting the client in identifying and moving into a suitable home and managing their tenancy agreement.
- Supporting and assisting occupants in activities that promote health and wellbeing, community engagement and recovery, financial budgeting, pursuing leisure and educational goals, social skills development, management of medication and appointments, and independent living skills and security.
- Processing of approved new referrals - following procedures for completing new assessments, risk assessments and creating individual person-centred service delivery plans.
- Follow procedures for conducting 6 monthly and annual reviews - or as and when required due to changes in individuals’ needs or circumstances, ensuring risk assessments and support delivery plans are kept up to date and are relevant.
- Completing the Outcomes Database and producing reports for internal/external purposes.
- Completion of individual support plans along with regular and timely reviews.
- Attendance at multiagency meetings to ensure the complex needs of the client are addressed appropriately.
- Maintaining records and statistics required to monitor and evaluate the effectiveness of the service.
- Data collection and appropriate recording and presentation for monitoring and Quality Assurance reports as directed by the Practice Leader.
- Follow procedures for general record keeping and information collection, saving and archiving - adhering to confidentiality and data protection policies.
- Implementing the Adferiad Equal Opportunities Policy and all other Adferiad policies and procedures.
- Working in accordance with Adferiad’s Recovery Programme.
