We are looking for an enthusiastic CPN/Mental Health Practitioner to work in our Older Adults Service. With an interest and passion for caring for patients with mental health needs including dementia. Previous experience is preferable however we welcome newly qualified applicants with a keen interest to learn and develop their skills.

You will work in our Community Mental Health team providing mental health care and treatment using appropriate pathways including the Dementia Living well Strategy in line with NICE guidelines.

A good understanding of Mental Health Framework and legislation in addition to Clinical Risk policies and procedures is desirable.

The role is full-time (37.5 hours per week) however interested part-time applicants will be considered.
- Participating as a member of a multi-disciplinary team managing a caseload of patients as a mental health practitioner.
- To undertake assessments of patients, involving their Carers and family members in the patient’s care and care plans.
- To beresponsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
- The post holder will direct and supervise Support Workers /or students.
- Post holder will provide advice to other members of the service and agencies on the needs of service users.
